2. Companies from the EU countries should fill in the VAT No.
3. For credit or debit card orders we do need additional confirmation from you, either:
   • by phoning us with your credit or debit card details
     or
   • by printing out the completed Order Form (A4) through your browser.
   • signing and dating your credit card details and order.
   • faxing or mailing the Order Form to us. (See below for our Company Information).

1. Your name, shipping address and telephone/fax number.
2. The billing address and telephone/fax number if different from above.
3. Your companies VAT No. if applicable.
4. Which products you wish to buy and their quantities.
5. Your preferred delivery method. It costs approximately:
   • $70 by fast Federal Express
   • $15 by standard air mail
   • £5 for delivery by standard mail inside the UK.

We will reply with the final price including discounts and shipping.
Products will be shipped on receipt of payment.
Payment can be made by international money order or direct bank transfer.
Credit/debit card payment is also possible.
All international payments (i.e. outside UK) should be made in US dollars.
